What do I need to do?
Firstly you will need to apply for the correct provisional entitlements to be added to your current licence, therefore you will need to fill out a D2 form, which can be filled out by yourself and a D4 form, which must be completed either by your GP or a medical centre.

Once the forms are completed, post both forms along with your current driving licence to the DVLA. Your licence should then be returned in approximately 6-10 weeks with the provisional entitlements added.

When you receive your licence back you can then book your multiple choice theory test and hazard perception.
